New Patient Information
Your First Visit
Experience the Edgewood Dental difference from the moment you walk in the door. One of our friendly team members will greet you warmly. We’ll answer any questions you have, show you around the office, and introduce you to any team members who are not busy attending to other patients. If you haven’t completed your new patient forms at home, we’ll take this opportunity to help you get this finalized. Next, you’ll receive a full diagnostic screening and professional teeth cleanings. The most important part of your first appointment and every appointment thereafter is the one-on-one consultation with Dr. Haughtington or Dr. Noreika. This is when we’ll partner with you to create your personalized treatment plan.
Prior to your first appointment, we’ll be happy to mail paper forms or make your new patient information documents available to you electronically. We know many patients prefer to complete these forms at home where they have access to all the information we may request. You can always feel free to contact us if you have questions while filling out your documents.

If you have dental insurance coverage, our knowledgeable team members will be happy to help you maximize this financial resource. We process and file claims for most providers to ensure you receive the benefits you deserve. We are also in-network providers with many of the popular dental insurance plans including:

At the time of your treatment, we’ll be happy to accept payment for your portion of the treatment cost in the form of cash, check, or major credit card. If you’d like to spread the cost of care out over several payments, we also work with CareCredit to provide low and no interest payment plans.
